Apprise
Apprise offers counseling on health insurance matters for older adults. Assistance can be provided with filing medical claims and policy comparisons and can be done via phone or in person by appointment.

Care Management
Consumers 60+ who have mental health issues or are developmentally disabled can also receive care management services through mental health or intellectual disabilities.

Congregate Meals
This service provides meals to individuals in group settings. Service includes at least one well balanced meal daily, up to five times a week, containing at least 1/3 of the Recommended Daily Allowances for nutritional standards.

Consumer Reimbursement - Pest Control / Fumigation
This is a consumer-directed service which provides reimbursement to individuals for services authorized in their care plan.

Home & Community Habilitation
This service addresses cleaning, organization, budgeting and more.

Legal
Legal services provide education, counseling, and training to individuals on a variety of legal matters that older adults are confronted with in regards to benefits, rights, wills, and taxes.

Medical Equipment & Supplies
Here you can find medical equipment and supplies based on the needs of the consumer and as prescribed by their Primary Care Physician.

Personal Care / Home Support
Personal care provides assistance with bathing, dressing, feeding, toileting, ambulating, application and removal of adaptive devices.

Recovery Specialist
A recovery specialist helps support a person in substance abuse recovery to overcome barriers to maintaining sobriety. They are also currently on a recovery path as well.

Senior Centers
Senior Centers are Community Centers where older adults can get together for social activities, recreation, education, creative arts, physical health programs, and nutritious meals.
