Mobile Meds

Overview

Mobile Meds provides assistance to individuals 18 years or older who have a diagnosed serious mental illness and a history of repeated inpatient admissions or state hospitalizations by providing support to manage medical and psychiatric medications while remaining in their homes or in the community.

Eligibility

This service is open to all adults, ages 18 and up.

The Mobile Medication Program  at RCC is a voluntary service offered to adults with a serious mental illness residing in Venango County. The program is designed to promote medication adherence and the reduction of crisis situations and/or re-hospitalizations. Also reaching out to consumers upon discharged from the hospital to provide support during their return to the community.    
a reconciliation of current medication is conducted in the home. Education is provided on the purpose, dose, frequency, and side effects of each medication, as well as symptoms to report and when to seek emergent care. In addition, consumers are assisted in managing appointments and their overall healthcare needs. The frequency and intensity of this service is based on the individual needs of each consumer.
